How to fill in a form on the mobile App (Our lives first)

Who can fill in a form?

All Users accounts can fill in a form on the mobile application.

How to proceed?

Step 1: Login to the mobile App

Login to your account with your username and password.

Step 2: Open the Forms section

Once in you account, click on the "Forms" section in the bottom of the page menu. 

Step 3: Choose the Form you want to fill

The list of available forms will appear. 

We choose "Our lives first" for the purpose of this guide.

Step 4: Create a New form

Click on "+New form" to create a new form, then select the type of form you wish to create. 

We choose "Safety green light" for the purpose of this guide.

Click on "Start".

Step 5: Confirm the intervention information

If you are on site, but there is no intervention, the Intervention site will be automatically detected and you will have to add the intervention number and participating company.

If you are on site and there is an existing intervention, you will have the option to select it.

Then, the intervention site, the intervention number and the participating company will automatically appear.

If you are not on an intervention site, you will receive the following message:

Step 6: Select the participants

Once you finished with the intervention details, you will have to add the collaborators.

To do so, click on "Add new collaborators".

You can then scan the badges of the collaborators to add them or select them from the Users Listing by clicking on the checkbox next to their names. Then click on "Done ()" on the top right of the window.

Confirm the list of participants by clicking on "Next".

Step 7: Fill in the form

After you are done with the intervention and collaborators details, fill in the form and click on "Finish".

Note: You can add photos by clicking on the camera icon.

Step 8: Confirmation

You will then receive a message confirming the successfully submitted form.

The form is now visible in the Forms listing.
