How to fill out the medical aptitude form on the platform

Who can fill out the medical aptitude form?

Company managers and Directeur EE can fill out the medical aptitude form.

How is it done?

Step 1: Access the platform

Begin by logging into your account on the IZI Safety account.

Step 2: Navigate to the document Listing

Once logged in, go to the "Documents" tab, located in the platform's navigation menu.

From there, select the "To transmit" section.

Step 3: Locate the account that requires completion of their medical fitness form

In the “To transmit” section, Locate the account you are looking for. You can use the "Filter" option if necessary.
Click on the account in question to display the list of documents and locate "Medical Fitness".

Step 4: Initiate

In the document details, on the right, you will notice a blue button.

Click on the blue button "Select file" to fill out the form.

Step 5: Fill out the form

Complete these 3 steps:

A- Choose the types of work for which the employee is deemed fit.
B- Insert the date of the employee's last medical visit.
C- Sign the document (you can redo the signature if needed), and submit the form.
